Apply to your general practitioner or any other specialist treating your child (paediatrician, neurologist, etc.);

A doctor will issue you an extract from the medical documents – referral for rehabilitation or sanatorium treatment (form 027/a), referral to a territorial sickness fund (form 070/a);

If a child is under 8 years or older with disability (cannot serve himself/herself), remember that the above-mentioned certificates must have the following note – a child requires mother or other accompanying person’s care;

Having obtained the doctor’s referral (F-027/a and 070/a), apply to the Territorial sickness fund, which will issue a certificate for a child to receive rehabilitation and sanatorium treatment, as well as a certificate for an accompanying person (if needed);

At the entry to the sanatorium, you must produce an identity document, F-02/a, certificate issued by the Sickness fund;
